When millions go without health care insurance in the US -- How is that not rationing health care?

Unless providers manage to find a way to be educated for free, receive all supplies and equipment free and have someone paying all of their basic living expenses so they can provide services for free, health care rationing will occur. It doesn't matter if it's a private HMO or PPO, lack of coverage or a government plan covering people. Rationing will occur. There simply isn't enough money in any of the systems to provide the highest level of care available through technology to every patient. Rationing occurs in nations with universal coverage, just in a different manner. People are either outright denied procedures or treatments are driven by cost as opposed to effectiveness. In other words, people may not receive the most effective treatment, but rather the least expensive. Nobody operating under any system in the year 2009 can deny rationing.
